One way that i have done this is by celebrating how the individuals efforts benefitted the team or the teams mission. The more you can point to how the team succeeded through someone's efforts the more you will shift the focus to the team.

This is a tough one. 1 of the most important step, I think is to walk the talk. As a servant leader, lead by example and demonstrates no task is above you. Continue to articulate how your team mate actions have an impact on others. Consistent communication at individual level and team level is important. Find alignment in individual recognition to teamwork through win-win philosophy.
