What I have used in some situations is to take a time out for a safety meeting, with the people involved. It is an incident review and we look at all of the contributing factors and at the same time review the procedures and safety protocols in place. The discord and potential safety failures could potentially result in decreased prodUctivity and an increase in cost of doing business. Sometimes, this could be addressed from an HR aspect but the underlying safety considerations must be addressed as part of a best solution.
