Formulate a transitional plan with all stake holders involved. This plan needs to include expectations on current responsibilities along with expectations for future roles. Set goals and hold the team accountable. Some of those expectations should include a communication plan to help employees stay on task during the transition. This communication plan should include an avenue to help any employee who needs help if the new process interferes with current business or becomes confusing. Become the transitional subject matter expert to help ease minds of those who struggle with change by explaining the process. Arm your team with facts that help explain the need to change. Encourage them through the change, stay positive during the transition.

For smooth communication during role changes, maintain transparency, regularly update everyone on changes, and establish clear points of contact. Encourage open dialogues to address concerns and use consistent channels for information-sharing.

We follow checklists during the transition. This is to ensure that nothing is left untouched. Since I am on top of everything, I also make sure that when I team member is assigned to a new assignment, I am there to guide and be consulted.

Just as the minutes in a city council meeting would recorded, always record these meetings with bullet points for reminders. Make attendance mandatory for these meetings. Make everyone feel welcome. Encourage questions, comments and concerns to be voiced. Have a secret comments box if necessary. Use team building exercises if necessary.
