It is very important to understand why the teammate is unable to complete the work, rather than rushing to blame or ostracise them. There could be valid reasons and empathy is key in this situations. If the reason is valid, then the best course of action is to divide the members work between existing teammates to ensure work is completed

In my experience, identifying the error can reveal the real problem to be tackled. It's not always the fault of the person being pointed out, but rather a process that is often flawed and brings an accumulation of errors that interfere with that person's activity. Talk to this person, review their goals and propose the use of tools that can improve their performance, courses or even a mentoring process to develop their shortcomings so far.

Firstly to figure out why that team mate is no able to cope up with the work. It is important to understand what professional or personal challenges that one may be facing due to which their work is being affected. Once that has been identified, the next step is to figure out is how you and the team can contribute to make the teammates work easier and openly discuss the possibile solutions. For eg one can guide him/her on how to increase efficiency or in the meantime the work can be distributed amongst the team members. It is about addressing the issue and coming together as a team to find a solution to help one of your fellow colleagues.
