Wow, this is a great one! I always perceived this could happen so when I sign up new clients, I tell them: 1. Any of us can reschedule 24 hours before the meeting. 2. Reschedules should not exceed more than 3 times. 3. Stick to the same time slot to minimize to and from communication. Example: 10 to 11 AM on every Monday. This has helped me keep reschedules below 5% in my business.

What I learnt from my 15years It exp, leading around 100 member team, below are few tips I would share: 1. At times if its happening, then no problem. But let this not become habit. 2. Proactively time shoudl be choosen which works for all. 3. Before setting meeting, confirm client availability. And prior to meeting, keep getting confirmation on availability. 4. Apply a train model if behaviour is repeting. That if we miss this meeting, then next will happen in upcoming week, and we will not be avable to discuss particular item, which delays entire process by 1week. If inefficiency is there in system, then consequences would come. But it can be handled effectively, with coordination & care. Thanks.
