I have done "post-mortems" with teams to reflect on what went well & what didn't. Taking time to slow down & examine what led to a certain outcome is important & often lost in the shuffle of busy schedules. But it allows you to capture what went well so you can repeat those actions & share them with others. And on the flip side, understanding what you could have done differently & brainstorming what you will do next time you encounter a certain scenario/challenge will help the team/team member be more prepared and have a more positive result. I would also add that creating culture where leaders come forward and share their failures will also setup a great example. In my expereince acknolwedging and celebrating failures more then wins is a great way to learn from them and enabling innovation and can do mindset despite the bumpy rides that may follow.
One thing I have found useful is that when a team fails, it's never the fault of everyone but the spirit intended is everyone takes the blame. I say, improvise the thought, the team leader should identify each person's capabilities and use that to move ahead and weak points to see what can be done to work on. ????
