In my experience the biggest challenge is capturing defect leakage inputs esp., defects leaked from SIT to UAT. This is the most important phase as you transition from internal environments to customer systems. Each defect leaked into UAT/production can have huge impact not just on your company's credibility but the cost of fixing it too. Hence, identifying that its a leaked defect, knowing why the defect leaked, RCA (root-cause-analysis), and a feedback loop the ensure it doesn't occur in future releases (or similar solutions you deliver to other customers) is important. This ensures you have a knowledge base and experience to deliver future solutions effectively. Many good tools other than excel are available to derive good inferences.
