To find the actual root cause you have to break this into 2 parts, with database proper monitoring tools. 1. Hardware/server-level issues:- Identify the bottleneck, Tune the database, Upgrade the hardware, and Use a caching layer. 2. Database level issues:- Normalize the database schema, Find and fix the Index level high-cost queries and optimize them, create required indexes, Write efficient SQL queries, and Use performance testing tools.

In my experience, I have found more about the issue by using the DAC(works best when we have Threadpool wait) and working my way out. important things which could be captured by being there are the wait stats

For sure is important to have a clear monitoring situation with historical trend about common metrics such as CPU, RAM, DiskIO, common query running time, ecc., then is possibile to analyze the difference between average trends and current issue situation. Also have a clear view of the last change performed inside the environment has a great value.
