The most critical thing in my mind to define the problem is to bring together the people who ultimately are responsible for solving the problem. Often times, this includes leaders, managers, and people who will execute solutions. These people need to be brought together with a solid plan on how to allow them to voice their opinions and needs, and then help them align on what the ultimate brief for finding solutions is.

People have different communication styles and comfort levels, even when we have a safe space for collaboration. This means we need to create options for how they contribute: some will volunteer ideas, others will need to be called on, and still others will want to contribute with stickies or even off-line. It's important to provide multiple modes and timelines for contributions to get the best ideas from all contributors.
