Depth of Field (often times just called DoF) can be achieved through different approaches. The "real" brute force method of actually ray traced camera rays being focused or defocused. This method is calculation intensive and introduces lots of noise. The second method, often used in realtime applications, is a (gaussian) blur filter that is controlled by a depth pass. Depth passes are essentially an image that contains distance values to the camera. "Bright" is usually far away, "dark" closer. This method is faster, doesnt introduce noise, but is very vulnerable to technical issues like depth sorting. Choosing the right method is essential and you have to always consider your end product.

When talking about depth or field or the Bokeh effect, it can also be achieved in post-production stage using a tool like nuke. We can render a Z depth pass or AOV in any renderer, and use that pass in nuke to create an accurate looking depth of field or bokeh effect. It can then be used for further compositing, overall decreasing the calculation a bit per frame during render time.

The more you manage to emulate a real camera, the better. We are very used to camera images being imperfect and a clean image will always look artificial to us. DoF and Motion Blur are just two tools of the set you can use. Also consider grain, over/underexposure, glare, bloom, lens flares, lens distortions, camera shake (3 dimensional!). Ian Huberts tutorials offer a great perspective into this from a Blender perspective.
