It depends on the degree of the "incident." The degree and formality of the incident investigation needs to be in line with the severity of the incident, otherwise you're just wasting resources and getting people angry. Severe incidents (even severe near-misses) should have a formal incident investigation. There are numerous acceptable methods, such as modified NFPA 941, 5-Whys, or other methods. Formal causal analysis should not be taken lightly: It can be expensive both in money and in time. Likewise, formal causal investigations will issue a report at the end which will include recommendations for corrective actions.

En lo particular y teóricamente, para mí se debe hacer un análisis en equipo con los involucrados del área donde ocurrió el incidente. Dicho análisis debe integrar los aspectos relacionados con los riesgos: capacitación, entorno, ambiente de trabajo, se?aléticas y comunicación efectivas, herramientas, condiciones de operaciones seguras y sistemas de emergencia, planes de contingencia y sensibilización del peligro, sin duda el uso de epp y finalmente puntualizar la o las causas raíz detectadas mediante un método estructurado y sistematizado (sin dejar la comunicación post ocurrencia).

In my view, the above can be followed by using the just culture model. Google that in images and you get a flowchart that run you through steps which helps to diagnose the situation objectively, rulling out biases and moving you to an actionable conclusion.

As an example, we can use automated software to gather all thoughts up from all parties engaged (Performing or operational personnel, area authority, and HSE) as initial. Then perform deep investigation to make an accurate root cause analysis by a collaborated committee then corrective action plan to be settled.

2 Focus on learning, not blaming

The second step to providing effective feedback is to focus on learning, not blaming. The purpose of feedback is not to assign fault or punish the workers who were involved in the incident, but to understand what went wrong and how to prevent it from recurring. In my experience this delivers results. Keep in mind right from the induction employees are informed of this ‘ideal world’ they have joined. With perfect procedures, managers and all the resources needed to create safety first. within minutes of joining the workplace what they were informed versus reality, I am reliably informed is dramatically different. Equally when incidents and near misses occur, Leaders response matters - focusing on learning not blaming will restore faith

Safety to be a priority and "Safety 1st " culture to be in an organisation . Safety should be in the form of compliance ,it should be in habits .Leaders should focus more in the feed forward rather than believing in feedback . After any safety incidents we must have a learning and it should be repeated. Union members and others concerned members to be a part of the emergency response team as well as the safety committee .

After an incident , there are tempers flaring unions trying to blame managements & vice versa , chaos on shop floor low morale of employees , statutory bodies jumping over the management Very high level of seasoned maturity is needed , leading from front , ensuring full & the best help is provided to the victim , medically , financially or morally Family has to be taken care & they have to feel that the organization is doing their best to provide needed resources The whole situation will calm down & the things would start getting back to usual Morale is the most important thing that needs to be taken care

Post an incident, what comes automatically are - Knee jerk reaction - Find someone to blame Management also plays a role in the above as they expect quick action and analysis even before the injured person (IP) gets medical support. - Remain calm - accident has happened already - Do not react - it increases complexities further. Act Just - Show utmost care for IP even though s/he may have been at fault - Put up a team to conduct investigation and risk based RCFA - Follow the consequence policy without exception like favouritism, performance bias, etc - Take measures to build trust - Increase Engagement with frontline, they are at risk mostly - Share the incident and learning widely - Most important is to pledge support incl financial
