Risk can be difficult to identify. Especially since those responsible for risk are often working in back office functions. I would suggest that risk should be identified using a systems approach, exploring each aspect using an appropriate model. Listen to those who work on the ‘front line’. Whether this be through focus groups, incident report analysis, survey etc. it is important to gather the perspective of this important group. Explore the process in small chunks as well as a whole system. Another important stage would be to look through the eyes of different stakeholders e.g. business manager, operations manager, staff, customer, executives etc.

The first thing to remember when considering any form of culture change work is that it takes time. Often attempts are cut short before they have been given the chance to really work. The first way to encourage a positive safety culture is to remove blame. In a blame culture people will be less inclined to report incidents due to fear of retribution. One way this can be done is by moving away from using root cause analysis to investigate incidents as this often leads to blame being apportioned to operators. Instead a systems thinking approach should be adopted. This does not mean that accountability and responsibility should not be taken for actions and it is important that this is considered as part of a wider investigation.

5 Monitor and evaluate safety performance

The fifth step to enhancing a safe manufacturing environment is to monitor and evaluate your safety performance regularly. This involves collecting and analyzing data on safety indicators, such as injury rates, incident reports, near misses, audits, inspections, and surveys.
