1 Failure Mode and Effects Analysis

Failure Mode and Effects Analysis (FMEA) is a systematic method of identifying and prioritizing the possible failure modes of a system, component, or process, and their effects on the system performance and functionality. FMEA can help to prevent or reduce failures, improve safety and quality, and reduce costs and downtime. FMEA involves three steps: identifying the failure modes, assessing their severity, occurrence, and detection, and assigning a risk priority number (RPN) to each failure mode. The RPN can help to rank the failure modes and determine the corrective actions.

The FMEA is very useful to identify how the system can fail and what are the effects of those failures. It won’t tell what are the causes of the failures though. For this reason, the FMEA is normally accompanied by a FTA, the latter being a suitable tool to identify the failures’ causes. These two tools complement each other pretty well. The Ishikawa Diagram can also be used to identify failure causes, alternatively or complementary to the FTA.

2 Fault Tree Analysis

Fault Tree Analysis (FTA) is a graphical method of modeling the logical relationships between the faults and failures of a system, and the events that cause or contribute to them. FTA can help to analyze the causes and consequences of system failures, quantify the probabilities of failure scenarios, and identify the critical components and events. FTA involves four steps: defining the top event or failure of interest, constructing the fault tree using symbols and logic gates, calculating the cut sets and minimal cut sets, and evaluating the fault tree metrics.

The FTA is an effective tool for finding out what is causing certain failures combining lower level events using boolean logic. It is not suited for investigating the consequences of these failures though (Use a FMEA instead if this is your goal).

3 Reliability Block Diagram

Reliability Block Diagram (RBD) is a graphical method of representing the structure and configuration of a system, and the dependencies and interactions of its components. RBD can help to calculate the reliability and availability of a system, analyze the effects of redundancy and parallelism, and optimize the system design and layout. RBD involves three steps: defining the system boundary and objective, drawing the blocks and connections using symbols and rules, and computing the reliability and availability formulas.

Keep in mind that a RBD focus on the funcional connections between elements, not the physycal ones. For example, if you need two generators to work together for a system to be kept “on”, the RBD should represent these generators in series with one another even if they are physically connected in parallel.

4 Markov Analysis

Markov Analysis is a mathematical method of modeling the dynamic behavior and state transitions of a system, and the probabilities of each state. Markov Analysis can help to simulate the performance and availability of a system over time, account for the effects of repair and maintenance, and compare different system configurations and policies. Markov Analysis involves three steps: defining the system states and transitions, constructing the transition matrix and diagram, and solving the matrix equations.

5 Bayesian Network

Bayesian Network (BN) is a probabilistic method of representing the causal and inferential relationships between the variables and parameters of a system, and the uncertainties and evidences associated with them. BN can help to update the reliability estimates of a system based on new data or information, incorporate expert knowledge and opinions, and perform sensitivity and decision analysis. BN involves three steps: defining the nodes and arcs using symbols and rules, assigning the prior and conditional probabilities to each node, and performing inference and analysis using algorithms.
