Missing to establish the escape point: Most of the analysts focus on the root cause and solution but they miss to find why the same was missed at first time at system level (Why not captured in FMEA /Product or Process Design) 3. Resolving Root Cause is Not Enough: While a lot of efforts are made to develop the Probable Cause in Fish bone, most focus only on Root Cause while other probable causes are not worked upon. It is important to validate that all other probable causes are also considered for Preventive Action to avoid future failures 4. Please Never Miss to Recognise team

It made me realize the value of structured approaches like the 5W2H method, which I'm now open to exploring. By incorporating 'who,' 'what,' 'when,' 'where,' 'why,' 'how,' and 'how much,' we can achieve precision in problem statements. This not only enhances our problem-solving abilities but also ensures everyone is on the same page, even when miles apart. In remote settings, staying open to effective methods is key to success.

Recognition of the problem to be addressed by the team requires proper analysis and thorough investigation of its root causes so as to not only state the accurate problem statement but also to convey its impact and demerits in the present time closely through the utilization of a well-developed interpretation. Lack of any one of these factors not only constitutes the creation of a wrong problem statement but also results in wastage of valuable time and respected efforts of the team members towards a problem which is not even crucial and neglects the actual critical problems which are still posing a threat to the advancement of the entire organization or team.

Poor problem description: A vague issue can lead to misdirection in problem solving. Insufficient root cause analysis: Focusing on symptoms, not the real cause, hinders effective solutions. Skipping steps: Each 8D step is vital; skipping any can lead to ineffective results. Inadequate corrective actions: Not ensuring the corrective actions truly solve the problem can cause new issues. Insufficient preventive measures: Failing to prevent recurrence can lead to the same problem arising again. Poor documentation: Not properly documenting learnings may cause recurring issues. Not involving the customer: Their perspective is crucial for successful problem resolution.

One of The common errors that has been observed is being subjective in the process of Root cause analysis & and having a prejudiced mindset. Hence it is very important to have a team representing the ideas from different angles & and a team leader who can see things from a bigger perspective.

Best practice for corrective action implementation is to first validate that the implemented action is actually effective and prevents or eliminates the problem. Then, at a later date, verify or confirm that the implemented action is still in use and effective.

As a best practice, part of the Preventive Action should include revising the FMEAs to include this root cause and implemented controls. Preventive Action should also include "Look Across" or "Fan Out" of the corrective action to all other assembly lines, work cells, processes, or factories that could be impacted by this root cause. This includes sharing the 8D with other departments, factories, and sites within the organization.

For root cause analyses that utilized 3L5Y, ensure there is an implemented, effective corrective action for each of the three (or more) root causes identified.
