What makes you passionate about Case Management?

The case manager is the ultimate patient advocate. They know more about the patient than anyone else on the healthcare team. They know the patient clinically, financially, demographically and how many stairs go to their front door! As a result they have the utmost accountability and responsibility to that patient. The best job I ever had as a nurse!!!

I am passionate about being a case manager, as my role is to ensure that patient and their families have a voice in their care. The patient is the missing link in improving the delivery of their care. Case managers improve communication between the healthcare team, the patient, the payer, and all stakeholders. We break down barriers, and when a service, procedure, or medication is not covered by the insurance company, we look at community resources to meet the patient's needs. We make sure the patient has a voice and is at the center of the healthcare team - we empower patients and their family to use their voices and participate in the plan of care to help them manage chronic conditions or come back after a catastrophic injury.

Case managers, social workers and discharge planners are some of the hardest working and most under-appreciated workers in healthcare. It has been enjoyable to see that the transition to value based care has brought to light the critical nnature of the case manager in the healthcare delivery process. For the most part case managers have shifted from within the hospital to outside the walls of the hospital as the payer is controlling the whole episode of care, not just the acute stay.

As a case management expert I get the outmost satisfaction in providing a helping hand to those in need by ensuring everyone gets a better chance to improve their lives. This also serves a purpose in our creation and makes the world a better place for everyone.

Case management is the most comprehensive and effective service available to people with multiple and/or serious issues. The collaboration, linkage, support and knowledge that a case manager provides is fundamental to recovery from any illness or addiction. Other services are incredibly important, but case management is the glue that helps hold the puzzle together and reinforce the skills learned in other treatments. It is integral to the client/member/patient understanding how all of their care is necessary, valuable, and a piece of what makes recovery work.

A fascination with solving complex problems and devising innovative solutions is another driving force behind a passion for case management. The role presents a myriad of challenges, from navigating limited resources and intricate systems to resolving ethical dilemmas and policy conflicts. Utilizing critical thinking, analytical skills, and decision-making abilities to navigate these hurdles is integral to securing the best possible outcomes for clients. The necessity for flexibility, adaptability, and innovation in response to evolving situations and needs underscores the dynamic and engaging nature of case management.

I love being a case manager because we genuinely make a difference in someone's life during their most challenging times. Helping clients navigate the complex insurance process, finding resources, and offering support for them and their families can significantly ease their burdens. Knowing that my work helps make a difficult time more manageable for others is incredibly rewarding.

Our complex & crowded health care systems now require either firm self-advocacy and/or professional advocacy in order to ask for and get what we need and deserve. Without this, in my experience-we often “fall through the cracks” or are put aside to be served later. This is one of the main reasons I serve adults as a professional care manager. I have the experience, credentials and assertiveness to help you navigate the health care terrain.

From my perspective, if we solve simple problems, It does not escalate to the complex. Simple solutions in healthcare is to recognize perception vs. reality of the individual patient and the population served.If the perception is not the reality, patient health outcomes will be negatively affected. If the perception is the reality then health systems have an opportunity to collaborate with patients and families and will make the job of the Case Manager easier and results driven

An innate curiosity and an eagerness to learn fuel the passion for case management. This field offers endless opportunities to broaden one's knowledge across various domains, including social work, psychology, healthcare, law, and education. Learning is not limited to formal training but extends through interactions with clients, colleagues, and partners, each offering unique insights and expertise. Engaging in professional development through training, certification, or research further enriches a case manager's skills and credibility, making the pursuit of knowledge a perpetual journey.

Making a positive change in someone's life. Being able to find ways to "yes" for the admission instead of no. Helping people who need help that have no community support.

"Why didn't I career-shift sooner?" This question haunts me every now and then. Making the switch has been a total game-changer for my career. Not only am I finally intriguing my brain with all the technical knowledge I've been craving after years in the psychosocial field, but it's also been a major self-discovery journey. This role has pushed me to level up my pressure-cooking skills, and I'm the meal ?? It taught me how to manage my own expectations, and helped me master the art of staying present. Plus, it's all about helping others in a crazy fast-paced and forever changeable environment. So yeah, my OG skills are getting a total upgrade, and I'm PUMPED to see where this path takes me next.

I find passion in case management because it allows me to directly impact individuals' lives by navigating complex systems and advocating for their needs. There's a sense of fulfillment in problem-solving and coordinating resources to create positive outcomes for those I work with. Collaborating with diverse stakeholders adds depth to my work, as I witness the holistic solutions we create together. Ultimately, my passion for case management stems from the meaningful impact I can make in supporting individuals and communities through challenging circumstances, contributing to their well-being and empowerment.

Grunds?tzlich ist es richtig, dass man als Case Manager auch viel au?erhalb seines eigenen Fachbereiches dazu lernt. Wichtig ist aber, dass man trotzdem das Netzwerk und damit die Experten der anderen Fachbereiche gezielt im Sachverhalt koordiniert und einsetzt. Dabei ist das Fachwissen über das eigenen Fachgebiet gar nicht entscheidend, da man dafür ja sein Netzwerk an Experten an. Jeder macht das, was er am besten kann! Der Case Manager koordiniert und steuert das Netzwerk und verliert dabei nicht das Ziel aus den Augen. Der Case Manager nimmt damit eine Lotsenfunktion wahr.

As a case manager working with the Intellectually Disabled, l enjoyed helping my Consumers and building relationships, to help create a “village.” One that supports and encourages the growth of the people on my caseload. But what l benefited from most was what my Consumers, their families and others who worked in the community taught me:unconditional love, acceptance, perseverance and a hopefulness that there is always something to be grateful for. And a loyalty to one another that was unbreakable. In my most frustrating moments, l had those examples from my Consumers to draw on. And it kept me going for 31 years??

The value placed on building meaningful relationships and collaborating with others is a cornerstone of the passion for case management. This role involves engaging with a wide array of stakeholders—clients, their families, service providers, agencies, and communities. Establishing trust, effective communication, and a supportive network is essential for fostering a respectful and empowering environment. Through these relationships, a case manager can effectively coordinate and integrate services, enhancing the overall impact of their interventions.

The professional relationship that is developed with a client, individual, group, community, or society is referred to as rapport buildupor which is called the relationship building process or bonding between two people to unknow to know. If you get into it, I think it's very simple to understand human trends through effective communication, emphasizing your valuable skills, and gaining trust from somebody along with acquiring their confidence that you are the one trying to support them and not someone who passes judgment.

There's no better way to get to know a person and what will truly benefit them until you collaborate with them to resolve a life issue. Together, you find resources, make connections, and exchange invaluable knowledge.

In my role as a case manager, you are responsible for forging connections and partnerships with a diverse range of stakeholders, including clients, families, providers, agencies, and communities. Your aim is to build trust, establish rapport, facilitate communication, and cultivate a supportive and respectful atmosphere.

I look at case management as the opportunity to organize someone else’s chaos and give it back to them in manageable chunks. It’s so heartwarming when you get to be part of someone’s journey as they learn to break down their goals into smaller action steps and then be able to actually see their progress.

Highly effective case managers share a common passion: others. Whether?helping patients, co-workers or other staff, they are constantly kind, committed and aware of the impact they have on those around them.

Case Management for me gives meaning to my life and I get job satisfaction knowing I made a life changing impact to an individual.

Finding personal meaning and purpose in one's work significantly contributes to the passion for case management. Empowering clients to achieve their goals and realize their potential provides a profound sense of fulfillment. Aligning one's efforts with personal values, beliefs, and a commitment to social change and justice enriches the professional experience. Reflecting on personal growth and the intrinsic rewards and challenges of the profession further deepens the connection to the work, providing a lasting sense of satisfaction and purpose.

Working in case management for many years has allowed me to help Individuals be the change in their lives. My personal belief is that most people want to make the changes that will improve their living situations. They just need help doing it. I find it so rewarding to do the work that I was meant to do. It gives me purpose.

Might we change our language from case to care management? It’s not a case we manage. People are not cases. We don’t manage people. It’s our care we manage. And not case loads. Rather our care capacity, our care resources, our care awareness and care conduct. (Thank you Talent Rusinga for the support of your request also.)

Passionate about working with individuals in a way which communicates respect for them and their lived experiences. Therefore It’s important not to treat or refer to the child / adult as a case but as a person. Perhaps the term case management has to be re-visited to promote a more person- centred approach. A case manager shouldn’t say they have 5 cases but 5 children.

1- I find great fulfillment in helping individuals navigate complex systems & overcome challenges. CM allows me to provide support, resources, & guidance to those in need, which can have a significant positive impact on their lives. 2- I am drawn to the interdisciplinary nature of CM. It requires collaboration with various professionals to ensure the holistic well-being of clients. This collaborative approach allows for a comprehensive and personalized approach to addressing clients' needs. 3- I am motivated by the opportunity to advocate for marginalized populations and promote social justice. CM allows me to work with diverse populations and address systemic barriers that individuals may face in accessing services & achieving their goals.

Case management is more than just documentation. It can effectively tell someone's story. Its important to recognize that each case is the history that has shaped a life. We have to be careful that we tell that story respectfully, accurately and with empathy knowing that at some point we've all been a "case" on someone's desk.

I am excited to share my personal and professional experience with case management. If you believe you know what someone wants to say, you must listen carefully and consider his or her stories, experiences, frustrations, agonies, struggles, and survival incidents indistinguishable from others. You should get into it. If you are someone who is interested in other people's stories without passing judgment and desires to attempt to understand what they are going through, you must wait until he or she has completed it, and you must show your interest by mentioning and responding.
