I think the biggest problem is that those who are doing the work are not checking their own bias. This means that there are huge challenges already built into the project so the key here is to have diverse teams at the onset of work, especially those who are doing work in the social issues space.

Take it slow and show you care. Do a lot of listening. Be there for them consistently and let them lead the way. Work actively on their effective empowerment. It's truly incredible when change finally happens, and people know they were an active part of it.
