What are the key roles and responsibilities in 8D problem solving?

8 Disciplines of Problem solving or what we call 8D in short is effective when performed through a CFT. There are actually 9 steps, including D0 and at every step, there is a need for various skills. Champion - The one who will lead the problem solving process. Team Members - Members who will be involved in collecting the data, providing inputs during brainstorming, Root cause analysis and Solution finding. These people shall be well versed with Process and Product. Also, team members should have good statistical analysis skills. Support - In case the problem is related to Supplier involve supplier person or if it's related to Design, involve Design team.

In 8D problem solving, a cross-functional team defines, contains, and analyzes the problem (D1-D4). They identify the root cause and implement corrective actions (D4-D5). Preventive measures are then established and validated (D6). Recognizing the team's effort (D7) and formalizing documentation, closure, and prevention (D8) complete the process. Each step involves specific responsibilities, ensuring a systematic approach to problem resolution and prevention. Simple: It's like assembling a skilled team (D1), investigating the issue (D2), containing it (D3), finding the root cause (D4), creating a lasting solution (D5), setting up defenses (D6), celebrating success (D7), and documenting the journey (D8) – all to conquer challenges! ?????????

The specific roles and responsibilities needed for an 8D problem-solving project will vary depending on the nature of the problem, the size and complexity of the team, and the resources available, The rules can be limited to a basic or traditional concept, so to speak, as follows: D0: Prepare and plan D1: Form a team D2: Describe the problem D3: Develop interim containment actions D4: Define and verify root causes D5: Develop and implement CA. D6: Implement PA. D7: Verify CAPA. D8: Congratulate the team and document and share the results. In addition to the basic key roles and responsibilities may can also add : * Subject matter experts * Facilitators * Scribes * Management sponsors

Selection of team members is very important. They shall be cross-functional and represent all areas of the problem and its impacted areas. Avoiding bias in selection is very important.

The 8D process initiates with a precise problem definition: articulating the issue with details on what, where, when, how, and how many, and measuring the current versus desired state gap. The leader ensures a clear, measurable problem statement; team members supply pertinent data. Following a structured approach, this step empowers the team to establish a solid foundation for effective problem-solving and subsequent stages in the 8D process.

In solving any problem, understanding is the first and most critical step. Without a proper understanding, we tend to throw ideas at a wall until something sticks, and communication breaks down. Clear definitions create a team. It doesn't matter if it is 6 people on a response team or an agent with a caller on the line--until that baseline is set, you are practically speaking two different languages.

In my experience….Without a well-defined problem, it's challenging to measure success or progress in finding a solution. It helps in clarifying the issue at hand, ensuring that everyone involved understands the problem in the same way. It allows you to concentrate on the root cause of the problem rather than addressing symptoms, leading to more effective solutions.

Defining problem clearly ensures everyone in team understands the issue and the 4W1H approach is a traditional and effective way to identify and display the problem.

The 8D process emphasizes the importance of a systematic approach to problem solving, continuous improvement, and a team-based effort to address issues. It's widely used in industries where quality control and problem resolution are critical, such as automotive manufacturing and healthcare.

"Containment in the 8D process is like the emergency brakes on a speeding car." Before you diagnose the engine's issue, you need to ensure the vehicle doesn't crash. In 2009, Toyota faced a massive recall due to unintended acceleration in some models. Immediate containment actions included halting sales of eight models, affecting approximately 57% of its inventory. The team leader's role? Orchestrate this rapid response. Team members? Execute and report on containment measures. It isn't just about damage control; it's about safeguarding reputation and trust while delving into root causes.

Containment is usually the first action that is to be done after identifying the problem. In order to stop the further potential spreading of the issue, we can: - Stop the production, - Verify the goods already packed in the warehouse, - Stop the delivery, - Verify when the product was delivered for the last time and where, etc. If your company is not a production-type one, then you do the same steps, but it is needed to adjust them so are applicable to your organization and type of industry.

Team members must allocate the necessary resources, such as manpower, equipment, or materials, to implement the containment actions effectively. Adequate resource allocation ensures that the containment measures are executed promptly and efficiently. In addition, the containment actions implemented should be thoroughly documented, including the specific steps taken, resources utilized, and the rationale behind each action. This documentation provides a clear record of the team's efforts and facilitates future analysis.

Containment is a vital risk management act in the occurrence of an event. For example if your PC gets infected maybe you clicked a link you trusted via email from your work or home office wherever. It is very important that you first disconnect your PC from the Internet including any other peripherals that may be connected primarily to the PC so as to contain the problem and not further spread it to other connected devices in your network.

RCA is the key stage of the process. I am involved in many 8D investigations with lots of ‘technical experts’ and the temptation is to jump to the ‘fix’ / corrective actions, until root cause(s) of the problem AND non- detection are fully defined and understood implementation of a quick fix might only address some aspects. Don’t be afraid to consider the people impact / influence such as thoroughly investigating human factor aspects. My advice, never jump to CA without doing a thorough RCA, consider all possibilities, quite often there are more than one issue. Never be afraid to go back to D3, containment might be costly and painful but needs to be robust whilst understanding we may not always get RCA ‘right first time’

In identifying the root cause(s) of a problem, beware of symptoms as efforts targeted at them will never guarantee a no reoccurrence. That’s why it’s important to start with the failure mode, breaking the problem into specific failure modes as much as possible. Before establishing the root cause(s) on a “failure mode”, it will be necessary to ask “if this (root cause) had not happened, would the problem have occurred?”. This is like working backwards the cause and effect logic.

Identifying the root cause is the backbone of 8D .The methodologies used for root cause identification includes 5 Y analysis,Ishikawa,Pareto law etc.This step is critical and identification of proper root cause helps in successful problem solving.The team members with their expertise identify the variables and parameters causing the issue/defect and together with the team leader translates the known factors in identifying the root cause.

This is perhaps my favorite step because I love analyzing and going into detail. Here we are asking ourselves and our team: Why did the problem happen? Why it was not noticed? What in the system failed to make us notice it? To deliver it (in case a product is non-conform)? We make a million questions, we give a million responses, and we see which ones are the closest to the root cause. And, as the Automotive industry is teaching us (that's why probably it is my favorite one), the Root Cause can never be a Human Factor. There is always something in the system and process that allows particular failure to happen.

In my experience, the this step really tests one’s ability to identify most appropriate root cause that is contributing highest to the defined problem. Here are some examples of room cause that can help you see how deeper one needs to get into : 1. Quality is bad: Manager is not engaged well enough with employee and not identifying and resolving challenges of employee on time 2. Customer is leaving : Client facing team member is disengaged as he is not receiving deserved appreciations, recognition and pay as he ideally should. 3. Compliance reports are taking too long to close : Individual is not onboarded well enough to fit in Organization’s culture. One should be extremely open for root cause analysis to be effective.

In my experience, before finalising any corrective action, one must ask : is it really solving the problem? Corrective action should be very closely related to root cause analysis. Here are some bad corrective actions which should be avoided: 1. Impart training on task when issue is with behaviour 2. Review 100% of tasks when issue is associated because of glitch in technology for only specific day Corrective actions should very much wise in nature. They should create investment of time for business and not waste of time.

D4 is root cause analysis. Many tools can be used there, the main one being a cause map where all the causality links must appeared. Technical and systemic root causes must be clearly indentified, and human contributions must be well analyzed to understanding all the performance influencing factors.

It is important to assess the potential risks associated with each proposed corrective action. This assessment helps in identifying and preparing contingency plans to mitigate any unforeseen challenges or obstacles that may arise during the implementation process. Clear performance metrics and key performance indicators (KPIs) should also be developed to monitor the effectiveness of the corrective actions. These metrics serve as benchmarks for evaluating the success of the implemented solutions and identifying areas for further improvement.

Developing Correctional Actions after a report must have been generated from the findings by the Opsec team. The detailed report generated from the root cause analysis will aid the security team correct and patch up any loopholes in the organization's systems or networks or workforce.

Beware of premature results of the 5 whys and the Ishikawa - teams should be patient and wise to systematically identify the root cause of a problem and develop effective solutions. Whether you are an individual or a team member, using a systematic approach to solve complex problems will enable you to produce confident, clear, and concise solutions.

Be prepared for corrective action to be met with resistance. Many will not be supportive of change. Ensuring that buy-in is obtained will better support the corrective action and ultimately allow the measurement of results to yield more unbiased information. While the leader may be responsible for overseeing and validating the implementation, the entire team should be a part of the process.

Corrective action is not the final action - it should be continuously reviewed for effectiveness, efficiency, and productivity especially when the corrective action is for a dynamic environment.

Implement corrective action or Verification Specialist: Ensures that the corrective actions are effective and that the problem is resolved.

This is the most important step in this Problem-Solving Method. Clear and defined preventive action can positively impact the whole organization. A failure that occurred in a specific process of our company can be a potential failure in others, and in this way, we are seeing the "gaps" in our system. We are implementing the preventive action even in the processes where we didn't have any (or similar) issues but after a failure comes, we notice that other processes are vulnerable, too.

Sustainable solutions will have effective recurrence prevention of the problem. It is one of the most important steps in problem-solving.

D6 An often overlooked component of preventing reoccurrence is the mechanisms of knowledge sharing. Usually during the problem-solving process only a few key teams within an organization are involved in identifying core issues and implementing solutions. Considering how you disseminate these solutions within the organization can be critical in preventing reoccurrences or in quickly identifying new issues related to the solution. Building organization resiliency not only prepares you for future problem solving but it also directly impacts company culture while providing a way to celebrate moving forward in D7

In the implementation of corrective Actions, you will be faced with immense resistance to change, you do not want to leave things to chance or creativity. Only a consistent, high-quality process will generate consistent, high-quality results when you are under pressure. Paying attention to the fundamentals of good troubleshooting is the basis for superior Problems. Management.

Besides a failure/problem that we had in the company, after solving it and implementing all the measures we agreed on as a team, it is time to recognize and celebrate the new lesson learned and a new system improvement. It is time to praise the good work and people who helped the company to achieve the targets and complete the tasks on time and without delay.

The D7 phase of the 8D process is a critical one to ensure that the morale and motivation of the team is aligned throughout the entire 8D process. The efforts of the team can and should be acknowledged across all steps of the 8D process by the leader, team for each other. Every step of the 8D process needs to be recognized, acknowledged and celebrated as it demonstrates the intent to move towards positive and long lasting change.

So much of Quality - Doing it the first time and all the time, within the context agreed, rewarding employees for motivation, employers should get it right. It is imperative to note that, rewards only work when they are meaningful and address the needs of the recipients.

The celebration in D7 can take many forms, from a simple team meeting to acknowledge the achievement to a more elaborate event such as a recognition ceremony or team-building activity. It can include verbal commendation, certificates of recognition, small rewards, or even a sincere "thank you." The choice of celebration is often tailored to the scale of the achievement and the preferences of the team.

The 8D problem-solving method involves defining the problem, forming a team, containing the issue, identifying the root cause, developing solutions, implementing them, preventing recurrence, and recognizing team efforts. It ensures systematic, efficient problem resolution while fostering team collaboration and continuous improvement. This method's disciplined approach and emphasis on continuous improvement drive organizational excellence, fostering a culture of problem-solving, innovation, and ultimately, heightened performance and customer satisfaction

The 8D problem-solving method is indeed effective in identifying, addressing, and preventing problems. However, it's primarily focused on corrective and preventive actions rather than explicitly addressing the consequences or damage that may have occurred as a result of the problem. To address the damage done by the problem, organizations may need to consider additional steps or a parallel process based on the nature and severity of the damage. While the 8D method doesn't explicitly cover these aspects, integrating them into the broader problem-solving and quality improvement framework is essential to completely address the full spectrum of issues that may arise.

The 8D is not only a problem-solving method, but it also makes us widen our horizons and look into the future, from the very first step. We understand what the problem is, we gather the team and define everyone's responsibility for each step, we follow up together the containment, we find and agree about the root cause. Then we define, implement, and follow the corrective and preventive actions, and see how they manifest in our processes. In the end, we are waiting for the final customer feedback and, hopefully, close the matter and have a lesson learned with many positive impacts on our system.

Every problem (regardless of its complexity) will always affect somebody (or something) through one consequence that will be the result of some failure. I like to mention the "San Gen Shu Gi" mindset from the master of problem solving Taichi Ohno who saved Toyota in the early 1950s from the bankruptcy. The 3 Gen are "Genba" (the place), "Genbutsu" (the object of the failure , the thin) and "Genjitsu" (the context of reality that created the failure conditions). Everytime somebody reached him asking for support he used to ask "Have you been there in the place where problem was identified?" or "Are you able to reproduce the problem?". If answer was "No" to one of these questions he used to reply "You do not have a problem to solve yet".

Ultimately, the fun part of the 8D process often comes from the sense of achievement and teamwork as the problem is successfully addressed and prevented from recurring. Also, the satisfaction of knowing that the organization has improved its processes and products can be a source of fulfillment for those involved.
