What do you do if your software project team and stakeholders are unaware of the risks and issues?

Its a good practice to use RAID register (Risk Actions Issues Decisions) for the project and update it on a weekly or fortnightly basis. Discussing the same on regular connects with the team allows everyone informed about the risks and work towards mitigating it. At the same time, since risks are thought through early on, there is a mitigation strategy at hand when risk becomes an issue.

Communication is key for project management. It could happen that though Manager has maintained Risk and Issues those need communicate regularly with Stakeholders. This communique could be through Focus meeting of Risk/Issues, Can be standing agenda of your dailies/weekly and your reports. This is equally important how manager elaborates impact to stakeholder and Action owner. These are key because most stakeholder look at any communique as Whats there for me.

We also have to educate them about the risks and issues by showing the impact they can have on our project. Communication about Risks or issues can be done in daily standup, Retrospective meetings, or sprint review meetings. Or by sharing Sprint reports like sprint commitment or sprint completion reports. By informing about the risks we also have to set up the person responsible who is going to work on the risks as well and needs to track those risks till those risks either retire or their impact becomes too low on the project. So simple process is to Educate, set a process for informing them, involve them in resolving those risks, and show measurable numbers about risk score of the project.

I have seen this occur and a lesson we learned was timeliness of that awareness being brought to all appropriate parties. The risk assessment needs to cover anything that has potential to impact the project's success, such as technical risks, resource constraints, scope creep, and external dependencies. Then collaboratively decide on options to address each area and communicate to the stakeholders the risks, the options to address and the teams recommendation on bet option to alleviate the risk. Most important part of communication is timely and regularly schedule check ins with the project team for updates. Then repeat the process if the risk is not addressed through the recommended approach.

This should be a continous process of reviewing the risk and issues to keep everyone aware and plan accordingly to keep the project and activities in perspective. Also, it will help setting up the appropriate expectation around the time, cost, budget, completion and plan rest of the dependant functions to cover everyone's interest.

Conduct brainstorming sessions with your team and stakeholders to identify potential risks from diverse perspectives. Maintain a shared document listing identified risks, their likelihood, impact, mitigation strategies, and assigned owners. Regularly report on risk status in meetings and project reports. Highlight critical risks and corresponding action plans. Establish clear communication channels for risk updates. Ensure stakeholders receive timely information on risks and how they're addressed. By implementing these processes, you create a culture of risk awareness, allowing proactive management of potential issues and fostering a collaborative approach to mitigating them.

Additionally, fostering a culture of open communication and collaboration is essential. Encourage team members to share their insights and concerns regarding risks openly. This can lead to a more comprehensive understanding of potential threats and innovative solutions to mitigate them. Regular team meetings can facilitate this collaborative approach and strengthen team cohesion.

It is also advisable to establish escalation process since in larger teams and stakeholder structures it is very possible that operational contract, like PM on client side, does not have mandate to solve issues. In this case you need to set up escalation process where you include communication to higher management.

Implement structured processes and procedures for identifying, assessing, and addressing risks and issues throughout the project lifecycle. Define clear protocols for risk identification methods, risk registers, issue tracking systems, and escalation procedures. Establish regular risk review meetings or checkpoints to systematically review and prioritize risks and issues, ensuring timely mitigation actions are taken.

The more the team size the more the need for not only communication but constant communication. Typical pyramid structure if used well, will serve the best for communication as people who communicate will need to understand the risks and issues thoroughly as they will need to answer questions from their team members. In this way you are making more people responsible for finding appropriate solutions to resolve or circumvent risks and issues. Bottomline being make more people in the team responsible for the project outcomes. . .

Regular 'two-way' communications have got a very important place in Project management. Please focus on the term 'Two-way' emphasized over here. Many people think that sending communication over emails or any other written channels may be enough. However, this is not the case - as emails or written communications often get overlooked or are interpreted correctly. Best way in my thought is to hold regular check-ins with requisite parties on RAID AND/OR receive a clear acknowledgement from all relevant parties in case the comms go written - to ensure that the process stays water-tight.

Communicating the risks and it's potential impact on a regular basis is important. The risk has to be communicated regularly via the project status report, progress report etc. In the regular routine project activities, risks may get ignored. It is critical to ensure that it is a part of most reports going upwards. The team has to have a pre-defined way of communicating risks upwards as well as downwards. Identifying and communicating risks should be encouraged and rewarded.

Foster open and transparent communication channels to encourage proactive sharing of risks and issues among team members and stakeholders. Schedule regular project status meetings, update reports, or dashboards to provide visibility into the current risk landscape and issue resolution progress. Encourage team members to report any potential risks or issues as soon as they arise to facilitate prompt action.

Unearthing Risks Together: Fostering Collaboration Organize workshops where the team and stakeholders brainstorm potential risks. Encourage diverse perspectives and open communication. Don't go it alone! Assess the likelihood and impact of identified risks together. This leverages the combined knowledge of everyone involved. Develop risk mitigation plans as a team. This fosters ownership and accountability for addressing potential issues. Keep everyone informed about project risks and ongoing mitigation efforts. Transparency builds trust and ensures everyone's on the same page. By working together, you can illuminate hidden risks, develop solutions, and ensure a smoother software development journey.

Collaboration is the key idea in handling Risks. As first step, the PM needs to collaborate with all relevant parties to identify and agree the RAIDs. As next step, collaboration is again required to keep assessing the RAID over the life of project and ensure that appropriate handling is being done. As last and most major step, it's actually the people in the team who actaully handle the risks and nullify those - so collaboration happens to be the most important ingredient of risk management. Also, the role of collaboration tools in handling the RAIDs is significant.

Assign owners to each risk and have them drive the monitoring and report on mitigation efforts as the team progresses through the project. If the risk is specific to a phase in the process, the risk can be closed out when appropriate, or can be tracked through the lifespan of the project, as required. Risk owners can estimate effort to track, mitigate, and report on risks to ensure impact on team capacity is understood. Each sprint planning session can touch on the risk mitigation efforts and allow team to contribute perspective, while also ensuring risk is understood and kept in mind for the team.

Promote a collaborative and supportive team environment where team members feel comfortable discussing and addressing risks and issues together. Encourage cross-functional collaboration and knowledge sharing to leverage diverse perspectives and expertise in identifying and mitigating risks effectively. Foster a culture of accountability and shared responsibility for managing project risks and issues across the entire team.

Develop actionable mitigation plans or contingency strategies to reduce the likelihood or impact of high-priority risks. Assign ownership for each risk or issue resolution task and track progress towards completion. Implement preventive measures or corrective actions to address underlying root causes and prevent recurrence.

*Planning risk responses should incorporate strategies for either positive or negative risks. -Negative risks need a mitigation plan, with strategies such as avoiding, transferring, mitigating , or only accept the negative risk. -Positive risks or opportunities need an aggrandized plan, with strategies such as exploiting, enhancing, sharing, or only accept the positive risk. -A fallback plan must be created, in the case of a risk that has occurred, and the primary response plan proves to be inadequate *A control risk process is also needed to monitor the implementation of risk responses, track the identified risks, analyze new risks, and determine the overall risk level of the project.

The project should have a RACI document with correct Risk Owner information. It has to be updated throughout the project lifecycle and shared with all stakeholders regularly so that risks can be converted into issues or avoided in the early stages of the project. Identified risk factors need to be addressed in team meetings as a priority and resolved before they become unavoidable.

When ensuring that software project teams and stakeholders are aware of risks and issues, it's crucial to adopt a "Review and Adapt" approach. Start with regular review meetings to assess the current risk landscape, involving all project participants. During these sessions, evaluate the effectiveness of existing risk management strategies and discuss any new risks that may have emerged. Encourage feedback to identify areas for improvement and adapt your risk management practices accordingly. This iterative process promotes a dynamic approach to risk management, keeping the project flexible and responsive to change, and helps maintain an up-to-date understanding of potential challenges across the team and stakeholders.

Quantify and Extrapolate A risk is something yet to happen, if one is able to quantify what would happen if the risk became an issue and what it would cost us to fix in the future by extrapolating in terms of financial impact it makes the decision making process easier and get the right attention for risk mitigation

Recognize and reward proactive risk identification and mitigation efforts to reinforce positive behavior. Continuously monitor and reassess the risk landscape throughout the project lifecycle, adapting your risk management strategies as needed to ensure project success.

Identify and prioritize potential risks, based on their impact and likelihood of occurrence, is part of every progetual meeting. Risks are monitored, not just controlled, and that means it is a continuous process.

To manage risks effectively, conduct a comprehensive risk assessment. Brainstorm with your team, use industry-standard checklists, and review historical data. Categorize the risks into technical, legal, financial, and operational groups, then prioritize them using a risk matrix. Document the assessment findings thoroughly for reference when developing and monitoring risk mitigation plans.

Experienced Project Management Team: Assemble a highly qualified team with expertise aligned with project complexity for effective management, clear communication, and sound decision-making. 3. Proactive Risk Identification and Management: Start by brainstorming "all the things I don't want to happen in my project", and then categorize the risks and apply appropriate analysis and mitigation techniques. 4.
