Replicate what works, and stay the course. And replace what doesn't with questions aligned with your mission critical goal. Once you have all that, you can focus on the delivery mechanism for collecting feedback. That might differ greatly based on your target audience. If you're clear on who that is, you can search for answers or advice.

If we are able to articulate and design a feedback and evaluation system based on bottom-up approach duly involving relevant stakeholders, it will add more value and the system is more realistic and user friendly. Hence while designing the system itself proper care has to be taken to make this more practical

Basically culture of the organization reflect the set of behaviours and values of individuals and their collective wisdom. This culture is backbone of any system to imbibe, assimilate and demonstrate righteousness in a true spirit for any organizational ecosystem. Transparency is one of these values and honesty is a steering to demonstrate transparency. Once we set a proper culture in the organization (addressing the root cause), symptoms like ineffective feedback etc can be easily handled
