Observation of quality control issues at every stage of a project clearly indicate that the quality management is not working. Need to review the quality management plan, execution strategies as well as monitoring processes and identify gaps, root causes, and find out if there is any trend or pattern. Based on the review outcome, the appropriate corrective actions need to be taken and lessons learnt should be shared with the broader team.

Perform the "5 whys" to get to the root cause of the issues. Then try to find out solutions for the root cause. It will be very enlightening to learn that sometimes root causes are far from obvious causes of issues in quality.

First of all, you need to understand the problem. You can create a postmortem using the 5 whys technique to detect the root cause. After that, depending on the root cause, you will need to improve unit tests, integration tests, end to end tests, or maybe implement or improve regression test automation.

Root cause is the initial step to consider when facing quality control issues. This is because, we cannot engage in corrective action or make decisions to revise the protocols before understanding where the issue emanated from. Thus, to make a better decision, a step by step root cause analysis should be done and be well documented, so that we can put controls and preventive measures in place.
