What do you do if your IT Operations are facing a major failure?

To handle a major IT failure, follow a structured response plan. Assess the situation, inform relevant stakeholders, activate your incident response team, diagnose the root cause, restore critical services, communicate transparently, implement permanent fixes, conduct a post-incident review, update documentation, and continuously monitor recurrence.

The key to identifying IT issues starts with triage of the basic failure points backed with proper monitoring and alerting system. While tools for monitoring and alerting are always there for initial investigation, the key lies in understanding the basic failure points and making sure all of them are functional before moving to a more complex solution. I have found that more often than not the problem lies in a basic configuration issue or a compatibility issue that has caused major outages.

I suggest you to make and design BCP in order to provide more reliability in your service. In my point of view we always face to probability of failure in Information technology operation and its inavoidable. but if you have Business continuty plan in you department or in your organization, you will be able to control impact domain of failure ,in addition it leads to that every member of your department knows about what should they do in that case.

Starting to assess the failure, discover the impact. Considering that this is a major - start to engage everyone that is needed to fix it, also proper management to have a clear and concise comunication flow. Companies should have a proper Sev1/Major procedure to follow in cases like this and avoid improvisations or best effort.

Support requests can be classified into different levels of severity according to their impact on the user and system. Critical issues that prevent users from using the product should be given immediate attention, while minor issues can be addressed at a later time. By categorizing the issues into various levels, support teams can prioritize their tasks and make sure that the most important issues are resolved first.

After identifying the cause of the problem, generate solution(s) and evaluate the pros/cons before implementing the most proper solution. In a major failure situation, implementing a solution without proper evaluation may cause further damages. Again, stay CALM and follow the proper steps.

Always have documentation for the solutions you implement. Both documentation on what you did and why you did it. If we are talking in regards to an exploit have the code to reproduce it for further analysis.

Always use original software and have a strong connection with the manufacturers of these products and their technical support. The first thing after reviewing internal expertise is to communicate with support for the manufacturers of the products that have a problem.

Communicate with the stakeholders prior to implementing any changes, i.e. solutions. The stakeholders do need to know if you are using the most proper troubleshooting steps. They may also help you understand the impacts when choosing diffeent solutions.

Some good techniques for root cause analysis are. 1. 5 Whys 2. Fisbone Diagrams 3. Change Analysis/Event Analysis 4. Fault Tree Analysis Drawing diagrams and providing supporting evidence help to validate hypothesis and avoid biases

[Continuing From my previous reply in 2.] After you gathered the data, reproduce the failure and find new instances of the failure if present. Having the documentation of the solution engineered/implemented is going to greatly change the outcome of this step in the process.

1. Find out what was the exception. and what is the out put. 2. Get the RCA of the issue from the top to bottom approach. 3. Based under the operation try to reset the default 4. Most import is the communication between different stakeholders

Quickly assess the situation, involve relevant stakeholders, and work to identify and address the root cause. Communication is key to keep all parties informed about the steps being taken to resolve the issue.

Use it as an opportunity to make sure it is as unlikely as possible to happen again. Continuous Improvement is a never ending goal.

A major failiure is no doubt a big problem. I have been promoting a 5-step problem solving process: 1. Define the problem 2. Evaluate the situation 3. Generate the solutions 4. Perform trade-off analysis 5. Implement, monitor and learn To prevent a major failure turning into an uncontrollable one due to misunderstanding the problem and applying the wrong solutions, stay calm and stick with the five steps. Also, have a documented and tested incident response plan that covers as many scenarios as possible handy.

By the time it is closed make sure to review the whole incident including all dimensions - technical, comunication, response time, etc... As soon as you have another case (it will happen) this process will be more mature since you performed a very good and reliable lessons learned. P.S.: Don′t forget to rehearse sev 1 process to make sure that main players are aware of it.
