We can use different tools and methods for this: Value Stream Mapping (VSM), Gemba Walks, 5 Whys, Fishbone Diagram (Ishikawa or Cause-and-Effect Diagram), Pareto Analysis, Kaizen (Continuous Improvement) Events, Check Sheets and Data Collection, Quality Circles, Visual Management, Process Observation and Time Studies. If it would be needed several methods to reconfirm the root cause - just do so.
