Do it right at first time, give sufficient time at initial phase of design. Standardize the process, list out possible corrections and have a eye on them before deliver to next phase.

I've been through this many times as a lead engineer. I will use circuits board design as an example. First, effective schematic capture and PCB design is analogous to swimming. That is, you should have a buddy to bounce ideas off of and cross check for errors. Second, I always, always use tabletop peer reviews. The best reviews are where work has been distributed ahead of time and participants come prepared. Believe it or not, a review checklist is super handy for catching loose ends not thought of by anyone else. Checklist examples can be fund with a simple Google search. I like revising them with lessons learned as designs and team learn the hard way.

When a circuit is not working as yourself or your colleagues expect then is there is something wrong with the fundamental assumptions that the design is based on. Circuits don't lie. Do experiments to push the circuits into area where is should perform badly or even fail. You have to change the conditions so that you get a wider view of the actual circuit operation. Even if this doesn't yield an answer directly the data gathered may be clues for other experts.
