What do you do if quality defects are jeopardizing your manufacturing process?

Identifying quality defects involves setting standards, inspecting products, analyzing data for patterns, finding root causes, continuous improvement, training staff, and evaluating suppliers.

Swiftly identify the root causes through thorough analysis. Implement immediate corrective actions to address the issues, such as adjusting equipment settings or enhancing quality control measures. Train staff on defect prevention techniques and encourage a culture of continuous improvement. Monitor the process closely to ensure sustained effectiveness and minimize future defects.

Beside quality processes (e.g., actual root cause analysis, effective CAPA,…etc), the most importantly is Quality culture, quality culture, and quality culture.

The manufacturing process producing the defected product must be analyzed carefully by a team work from all relevant functions, all process variables should be invistigated through data collection on the used raw material, human working on the process, the method used, machine processing parameters, work environment, defected product inspection and validate measurements. The most important is Gemba walk, you must observe the process on the real work place, collect information, discuss with first line staff, show respect and appreciation. It is crucial at this step to dissiminate the team mission, which is mainly to find out the root causes of defects and prevent its recurrence, not to blame anyone.

From my experience, the first step in tackling quality defects in a manufacturing process is to meticulously identify and document the issues. This involves conducting a thorough inspection of the products and processes to pinpoint where the defects are occurring. Using tools like checklists and defect tracking software can significantly aid in this process. For instance, I found that setting up a team specifically for quality inspection helped us to quickly identify patterns in defects, which could indicate deeper systemic issues.

Analyzing causes involves investigating factors such as equipment malfunction, human error, material issues, process deviations, environmental factors, and systemic issues to pinpoint why defects occur in the manufacturing process.

All the data and information collected should be monetarized on priority basis to analyze and identify the root cause. 07 QC tools are very effective to analyze and identify the solution of any problem.

Once the issues are identified, the next step is to analyze their root causes. Techniques such as the Five Whys or Fishbone Diagrams have been particularly useful in my experience for digging deeper into why defects are occurring. In one case, using the Five Whys helped us uncover that a recurring defect was due to an outdated calibration of a key machine, which wasn't immediately obvious at first glance.

All data, information and observation collected into the above step should be throughly analyzed using different root cause analysis tools, Pareto, 5Whys, Fishbone, ...etc. don't be fooled by symptoms that attract you far from the real root causes, if possible try to validate the agreed upon root causes through data collection for verification. All root cause analysis journey is a collective team work effort.

When it's time to analyze the causes of the identified defects, it's a common practice to get the team together for a brainstorming session and dive into the root of these defects. We'll use the Ishikawa Diagram, a great tool that sorts our ideas into neat categories like methods, machinery, materials, you name it. This way, we're sure to cover all bases systematically, but in a friendly, team-focused manner. It’s all about finding those sustainable solutions in a way that makes everyone feel part of the process.

The main objective of the quick immediate fixes is to contain any further losses you may cause to your customer, this could be accomplished through the rework, remanufacturing, outsource, increase stock level temporarely,.....etc. These actions just taken to correct and regulate the defected output as its taken against the symptoms not the source disease (root cause) but will never prevent recurrence of the same defect in future.

Implementing fixes requires a strategic approach to ensure that solutions address the root causes effectively. In my experience, creating cross-functional teams to develop and oversee the implementation of solutions ensures that fixes are comprehensive and sustainable. For example, when we discovered a critical defect related to material quality, we worked closely with our suppliers to develop a stricter quality control process, significantly reducing defect rates.

Managing quality defects in manufacturing 1. **Root Cause Analysis:** Identify why defects are happening, which may entail scrutinizing equipment sources of contamination (SOC) and Hard-To-Reach (HTR), processes, materials, or human elements. 2. **Continuous Monitoring:** Establish systems to consistently monitor quality throughout manufacturing, including regular inspections, quality checks like variable and attribute checks. 3. **Quality Assurance Procedures:** Develop and enforce quality assurance procedures to ensure consistency and reliability, including standards setting, conducting audits like Process-Based Quality Assurance (PBQA) post-maintenance or size/product changeovers, and providing ongoing employee training.

Implement fixes by addressing root causes, optimizing processes, enhancing quality control, providing training, collaborating with suppliers, and continuously monitoring effectiveness.

Ante la amenaza que representan los defectos de calidad para los procesos de fabricación, debemos centrarnos en identificar de manera exhaustiva todos los problemas presentes. Además de abordar soluciones inmediatas, es fundamental dirigir la atención hacia mejoras a largo plazo en los procesos. La mejora continua es un principio fundamental de la gestión de calidad, comenzando por planificar nuevas estrategias, implementarlas, verificar su efectividad y actuar en consecuencia, además de actualizar los protocolos de control de calidad, invertir en nuevas tecnologías o redes y dise?ar componentes del producto. Al potenciar los procesos de esta manera, se puede reducir la probabilidad de futuros defectos y aumentar la eficiencia general.

Continuous improvement is key to mitigating defects in the long term. I've found that adopting methodologies like Lean Manufacturing and Six Sigma can systematically reduce waste and improve quality. Implementing regular review cycles of processes and performance metrics allows for ongoing optimization. One of the most impactful changes we made was introducing regular Kaizen (continuous improvement) sessions, leading to significant efficiency gains and quality improvements.

Embracing an agile mindset is crucial for long-term improvements in manufacturing processes. This approach enables you to adapt quickly, experiment with new methods, and learn from each iteration. By updating quality control protocols, incorporating new technologies, or even redesigning product components through this agile lens, you're not just fixing issues temporarily. You're fostering a culture of continuous improvement, crucial for quality management. This agility reduces future defects and boosts overall efficiency, keeping your processes ahead of the curve.

Improve the Process based on the new processes, tools and methods. Focus in long-term improvement of the process such as new technology, processes and procedures.

Improving the manufacturing process involves streamlining workflows, implementing automation, standardizing procedures, continuous monitoring, lean principles, technology integration, cross-functional collaboration, and empowering employees.

Personnel training is critical to ensuring that quality improvements are sustained. Training programs should not only focus on the specific skills required to reduce defects but also on fostering a culture of quality awareness throughout the organization. In my experience, creating a mentorship program where experienced workers train newer employees resulted in a more knowledgeable workforce and a noticeable decrease in quality defects.

Ante los desafíos de calidad en el proceso de fabricación, enfocarse en el entrenamiento del personal es fundamental para mantener altos estándares de calidad. Este entrenamiento no solo debe abarcar habilidades específicas para cada rol, sino también comprender la importancia de la gestión de calidad. Es crucial capacitar a los empleados para que puedan identificar posibles problemas de calidad y comprendan los procedimientos para informar y abordarlos. Un equipo bien entrenado es nuestra primera línea de defensa contra los defectos de calidad en los procesos de fabricación y en las Auditorias.

Training personnel involves clarifying roles, providing technical and quality standards training, ensuring safety, promoting continuous learning, empowering employees, and gathering feedback.

Sustenance of action is the most important step in solving a problem. This sometimes may get over looked, which will result in re-occurrence of the problem. Action which is previously implemented can be broadly categorized as process side action and inspection side action. Characteristics of the improved process/ parameter can be considered as monitoring factor for this stage. When this continuous flowing data is plotted into SPC tool like run chart this will also give you valuable insights of health of improved process.

Here first we will put fire wall station to make customer assured and then revisit complete quality system and work on the gap analysis. Once situation is under control sustenance through audit mechanism will be adhered. Meanwhile will ensure the morale of team will be boosted.
