Initially, 2 things are vital when a(n) (interventional) procedure isn't going as planned. 1- Is it responsible to continue or try again? If not, the procedure should be abandoned at that time. 2- Was there any damage done or are there possible risks for the patient? These will have to be managed ASAP.

I personally inform the patient on the spot and update him/ her on the management of the issue that occurred. With radiological procedures the patient usually is conscious and involved, so you will have to communicate immediately.

It's so important to be open about things that go wrong or mistakes we make in the medical field. We all do it and hiding them or shaming others for them will only make working conditions worse and prevent us from learning and becoming better physicians. When honest and open, I have never encountered a patient, who reacted badly to anything going different from what was expected.
