What are the current gaps and opportunities for AI in HIT research and innovation?

AI applications in HIT are rapidly evolving and hold immense potential. In EHRs, AI can streamline data entry, reducing the administrative burden on healthcare providers. Clinical decision support systems powered by AI can analyze vast patient data to suggest more accurate diagnoses and treatment plans. In medical imaging, AI's image recognition capabilities aid radiologists in identifying abnormalities swiftly. Natural language processing enhances the extraction of valuable information from unstructured clinical notes, aiding in research and patient care. Telehealth powered by AI enables remote monitoring, extending healthcare access to underserved areas.

I think, gaps include the need for robust data collection and accessibility, ethical considerations regarding patient privacy and security, and the challenge of integrating AI into existing healthcare systems. On the other hand, there are promising opportunities to leverage AI for predictive analytics, early disease detection, precision medicine, and improving patient outcomes.

From my experiences at University of Rochester Medical Center, I have seen gaps in healthcare data management. Medical equipments capture valuable information related to patients and diseases but this is all stored in different formats. Lot of data cleaning and preprocessing is required before they can make sense and tell a story.

There have been several news stories of breaches, ransomware, etc and security remains a challenge. Also, AI will replicate patterns it is fed so there are various biases that AI may replicate, even scale. If historical data have been missing certain populations or have wrongly labeled data (if definitions have change or if EHR data have misdiagnoses) then AI cannot fix that bad data, rather will be faulty due to "GIGO" or garbage in, garbage out. High quality data are essential to good AI.

In this context, AI's ability to differentiate patterns can aid in the early detection of new onset conditions, flag exacerbations and decompensation, and lead to more tailored treatment and management approaches for patients, taking into account their individual backgrounds and "baselines". By continuously learning from vast amounts of patient data, validated models in HIT will soon provide HCPs with insights into evolving diseases & symptom profiles, hopefully leading to more efficient & effective patient care.

I read a Forbes article on the Future of Retail, which correlated so much with Healthcare. Here is my take on latest trends for AI future in HIT. -Multimodal AI: Models handling text, images, and video improve diagnostics and patient monitoring. - Smaller Language Models: Efficient, open-source models outperform larger ones. - Customized Local Models: Tailored for specific tasks, enhancing accuracy and privacy. - Virtual Agents: AI-driven agents handle queries and admin tasks. - Ethical Concerns: Privacy, bias, and transparency remain critical. These are apart from the usual areas of AI impact such as Drug discovery, personalized care, clinical decision support, etc which are already disrupting the current medical and admin healthcare.

Based on my numerous conversations with healthcare CIOs, a main challenge towards AI is a sense of ownership from the vendors/ technology providers. Healthcare providers want more partnerships, a sense of responsibility, and a co-development model from technology vendors rather than simply entering a business contract for SaaS services offering AI. The vendors need to be more transparent, offering customizations if needed, and most importantly guide the healthcare providers on which AI use case best fits their organization's current IT infrastructure and their goals. Unfortunately, this aspect is undermined in most of the AI solution buying process.

By addressing these issues, healthcare organizations can foster a positive environment for the acceptance and adoption of AI systems, leading to improved patient care and outcomes. Meanwhile, ensuring collaboration with the domain experts and other necessary stakeholders in the implementation of healthcare data, and the development and deployment of AI systems for the healthcare sector can facilitate a more positive attitude towards the systems.

This bias can stem from the data used to train AI models, the design of the AI system itself, or the societal and historical inequities reflected in the training data. AI systems learn to make decisions based on the patterns they identify in the data they are trained on, so if the data contains biases, the AI system will likely replicate or even amplify those biases in its outputs.
