It can be the either a change in the client's requirements or an increase in number of features required .

Effective communications seems to become a lost skill. A few recommendations: * Do not rely solely on digital communications as it often becomes a crutch. * Be open to criticism and accept it as a gift, not an attack. * Post mortem all projects to understand where inefficiencies existed * Ensure the project was effective after delivery. This one is often a big miss as it turns over to operations, few people actually follow up to see if delivery is fitting purpose. This becomes even more apparent when building or retrofitting a campus and the design has a major flaw that is repeated due to lost communications.

Quality issues are when the project fails to meet the quality standards or expectations of the client or the stakeholders. Quality issues can result from poor design, defective materials, faulty installation, human errors, or inadequate testing or inspection.
