You need CAD and CAM software. Sometimes these are combined in a single program but that's not always the case, nor is it necessarily required. You can type G-code in a text editor. Granted, it would not be very efficient, it's definitely possible. True 3D models will require a minimum 4 axis machine to create. A 2D model with depth, or 2.5D model can be created with a 3 axis machine. 2x2.5D models can be combined after machining, to create a fully 3D finished project. You will need to understand models and vectors and the software required to create or manipulate them, combined and individually. You also need to understand tooling and tool paths. With your designs complete, you can look to export your code and machine your parts.
