However, if that does not meet the outcomes needed, and the incentives are not enough to encourage compliance, then being consistent in the accountability is needed.

Review and adapt should be ongoing with everything. If you do not review, adapt, repeat, then you run the risk of becoming outdated, stagnant, and potentially failure. Always look for ways to improve. Employees are more engaged in a learning organization, not one that still employs archaic policy and disengages their people.
