Clear documentation and regular validation of requirements are crucial. This approach has consistently helped me ensure that testing aligns with both user needs and project goals.

Yet we often find ourselves in situations where, despite spending tremendous time and resources, there's a mismatch between what has been designed and what is actually needed. One thing that has worked for my team is reviewing the business requirements early and often (pre-refine). This allows us to ask questions for clarification, resolve ambiguities, and get the User Story in a better state before our whole team refinement sessions. In a lot of cases, asking questions leads to other questions being asked and a deeper dive into the User Story, allowing for multiple avenues and lines of thinking to spark engagement in others.
