What are the best practices for creating and presenting a fishbone QHSE diagram to stakeholders?

In my experience about creating Fishbone is basically for quality problem/process improvement issues which can be resolved with systematic approach. in addition to QHSE is challenging wherein standard approach of fishbone is quite confusing as the level of problem is broadened to QHSE. just like categories for fishbone you can use 6M or service approach but in addition you can take into consideration like workers, environmental aspects, HIRA, safety related risks and opportunities, legal and other requirements etc which will help all stakeholders to think and resolve issues taking not only quality but also health, safety and environment in which context situation arises. rest is tool which you can effectively use.
