Yes, all of the above are important, but you have to remember to structure the questions in a way that allows the respondents to give the most accurate answers based on their familiarity with feedback. For instance, if the respondents are young and do not have a lot of experience giving detailed written feedback to open ended questions, you might want to use rating scales instead and vice versa.
