It needs to be part of a strategy around automation of security response. just logging a user's actions in cloudtrail is useless is no one is looking proactively at the logs. Enabling Cloud trail is the basic first step and for compliant solutions such as Center for Internet Security (CIS) benchmarks it is mandatory. In my previous work I have used Cloud Watch Log filters to capture particular events and alarm on them. This provides some automation of notification but this can be taken further with automatic remediation through Lambda if suspicious activity is detected

I agree with others that centralising Identity is a very important way to make it manageable in the long term. I know companies that use their Microsoft Entra ID (Formerly known as Azure AD) with AWS IAM Identity Center (formerly known as AWS SSO) to allow access to be controlled centrally and assigned based on groups. AWS Best practice is to use groups rather than assigning policies to users directly and this works well with a Federated identity where the groups can be mapped to specific roles or Permission sets in AWS IAM Identity Center. And as long as the user lifecycle is appropriately maintained in the identity provider (ie users disabled/deleted when they leave or change roles) it is more secure than trying to maintain IAM users
