You really do need to make sure you have the other persons attention when you're talking to them. Establish eye contact prior to having a conversation, so the other party knows you are talking, or wish to talk to them.

Any incident response on the ground is going to witnessed by those nearby. The first thought is to rush in a help, and then after that the thinking stops. The first action needs to be taught in initial training, of STOP, Am I safe? Then, What's just happened? followed by how do make this situation safe? Then go and see if you can help. Or more simply, STOP, ASSESS, PLAN.
