Also, I have seen too often people hiding behind the process, not taking responsibility for their actions but following the process to the dot. Don't get me wrong - the process should help as an essential known path to take, avoiding repeating the same mistakes or reinventing the wheel. But process is not set in stone to justify switching off common sense or reason, inability to recognize that the situation may require a different, perhaps unpredicted step or a modified approach. Too much faith put in process may run a risk of "operation was successful, but the patient died."
