Good data collection is critical to proper evaluation of current practices. If a process is partially undocumented and unable to be reported upon it is VERY difficult to assess how it can be improved. We have seen quite a bit of client optimization of case management practices simply by putting their existing case management processes into a structured workflow tool (even if some of the steps are just acknowledging "yes this is done"). Being able to capture data is the first step to uncovering process bottlenecks and improving indicators of success.
The second step to update and revise your Case Management practices is to review the new evidence and feedback. What are the latest findings, recommendations, and guidelines from the relevant research, literature, and experts in your field? What are the emerging trends, issues, and needs of your clients and communities? What are the feedback, suggestions, and complaints from your clients, colleagues, and stakeholders? How do they align or differ from your current practices? By analyzing these sources, you can determine what works, what doesn't, and what needs to change.
The third step to update and revise your Case Management practices is to compare and contrast your practices with the new evidence and feedback. How do your practices match or deviate from the new evidence and feedback? What are the gaps, discrepancies, or inconsistencies between them? What are the reasons, benefits, or risks of following or ignoring the new evidence and feedback? What are the implications, consequences, or trade-offs of changing or maintaining your practices? By evaluating these aspects, you can weigh the pros and cons of different options.

How will you adapt to the challenges, uncertainties, and opportunities that may arise during the process? How will you coordinate and collaborate with your clients, colleagues, and stakeholders to ensure their involvement, support, and satisfaction? How will you document and report the changes and their results? By doing these actions, you can ensure the success of the changes.
The sixth step to update and revise your Case Management practices is to evaluate the changes. How will you assess the outcomes, impacts, and indicators of the changes? How will you compare them with your baseline, goals, and objectives? How will you collect, analyze, and interpret the data and feedback from your clients, colleagues, and stakeholders? By answering these questions, you can close the loop and start the cycle again.
