As far as I'm concerned, it's essential to track - Guides and studies on process safety best practices. - Monitoring the safety performance of processes using specific indicators is essential. - Innovation in process technology, such as the elimination of biocidal products and optimization of production systems. - Adoption of digital technologies to improve efficiency and safety in operations. - Training of employees in risk management and emergency procedures - Regular risk analysis is used to identify potential hazards - Participation in conferences and workshops on chemical process safety

I would recommend following standards and regulation in all our life tasks, example NFPA norms should be followed for materials selection which means the materials are complied with specification and can resist for fire, temperature, hard conditions and very safe to be installed and used. If we follow safety OSHA instructions for our work environment, materials selections, it means we are Woking in low-risk environment and at the end of the day we can return to our home safely.

API standards to be followed for designing systems and plants. Also need to think beyond standards as per case basis chemical engineering aspects and real life practical knowledge need to apply.

In my experience, unlearn is equally important as Learning new things. Unlearn practices that are not so great and Learn and Share best practices. Developing right questions is important rather than running harder to get answers. If I may share more reflections to quoted examples, the same mistakes / findings seen in Bhopal Gas Disaster (1984) are seen in BP Texas City Refinery Explosion (2005) even after decades of learning, developing new technologies and stressing on Process Safety. Are we really learning or trying to manage the Process Safety? One should feel intrinsically save by following best guidelines and practices to keep Safety as First Priority.

In my experience to learn from case studies and previous incidents will help us a lot to understand the root cause of the previous mistakes and how we can avoid them as much as possible.

Learning from case studies and incidents is a critical practice to integrate process safety in all aspects of process design. In addition, I believe in applying dynamic simulation to exercise and observe the consequences of various system failures and maloperation at almost no cost if the engineer can prepare the simulation himself/herself.
