I guess a good thing to do is outline what is OUT OF SCOPE for this endeavour and what issues/difficulties it won't solve/address, as well as what information it doesn't contain. Another thing is clarity about ownership/update permissions and how they should be done - in a product workflow, there are many roles involved, so everyone should know what to do if they want to propose an update/change, and who owns the documentation. Of course, it makes sense that the designer is the owner but in a process where diverse roles collaborate, lines can sometimes get blurry, and this is also very much organisation/context-dependent.

Choosing the format for the design document is nothing different than choosing the right layout for the screen, placing the elements with accurate spacing, and giving the primary colour to the CTA. We all do this for a reason so that the user finds the information quickly and can navigate through the screens easily i.e. achieve his/her goal. Similarly, the right format for a particular document needs to serve the purpose For eg, a Design brief is a document which is a one-stop shop document for any product feature, which covers its problem statement, why and how we need to solve this problem, persona details, what is the end vision solution, user research details, timeline and milestones, key user flows. Best thing to do is just ask. If you are making documentation for someone else just ask them what format would make it easiest for them to understand/use. When I ask I like to give examples like Figma or Powerpoint because sometimes it's their first time being asked or thinking about a design documentation. The other reason I like to give examples instead of letting their imagination run wild is that we, as the designers, are more likely than not the ones who have to upkeep and maintain the document. If I can't easily make adjusts as needed then I am setting the documentation up for failure.
