It is critical to avoid cognitive dissonance/confirmation bias, groupthink, conflicts of interest, and wishful thinking by including Devil’s Advocates, Vulnerability Assessors, and other skeptical honest brokers to question everything, especially at the test design and interpretation stages. Externals with no skin in the game are best but you can instead appoint internal DAs if external people aren’t practical.

If the pilot includes impacts on people, particularly vulnerable / marginalized populations, including explicit benefit/risk assessments, including metrics is essential - the pilot could be a "success" but a subset of your target population may be made more vulnerable.
