Impediments or bottlenecks can be of two nature internal or external - for internal impediments - we can do RCA by using 5Ys or Fish bone and for external bottlenecks which includes external blockages due to dependency on external team - Scrum of Scrum is good ceremony for eliminating such dependencies

In my opinion, of course the team must be self-organized, but if your project has a structure in which the Project or Program Manager figure is also there, these figures must work together with the Scrum Master and take the lead in resolving issues and impediments in order to help the team to delivery what was planned during the PI, Sprint, Realeses,....

Some problems that may lead to bottlenecks tend to hide in the responsibility transfer areas between departments. That's why it's crucial not only to find and eliminate the bottleneck itself but also to review the entire process, identify the root cause, and address it.

This perspective may be a little different, but encompasses all that has been mentioned above. If an Impediment just won't go away, the Team probably hasn't identified the root cause and should begin the A3 process.Quite often impediments will be related to product management and continuous collaboration with stakeholders will help to address it quickly.A clear Sprint Goal is an useful instrument to determine if something is truly an impediment.
