When it comes to organizational culture, There is so much to learn and act on in the comments employees leave. As a leader, I always set aside time to read each and every comment an employee in my team leaves, which has provided deep insights into what is working well and what needs to be fixed.

You can implement a feedback loop at any level to seek for continuous improvement. To be efficent the loop must be done on regular basis at short time intervale. As purpose to capture data, you need to collect it from the doers, not from someone in between. I strongly suggest to setup at minima a monthly loop at team level and a trimester loop at department level (or at scale project). Good! Now you have organized it and you start collecting data, the next step is to improve the system. So every time you make a meeting to close the loop, find an action to solve the biggest impediment in the system. Feedback loop is useless without any improvement then.
