1 Roles and Responsibilities

As a hospice liaison, there are a variety of tasks that aim to facilitate the referral, admission, and transition of patients to hospice care. These duties include educating patients, families, and health care staff on the benefits and services of hospice care, assessing the eligibility and needs of patients for hospice care, coordinating the admission process, communicating and collaborating with hospice staff and other health care professionals, providing ongoing support and follow-up to patients and families throughout the hospice journey, and documenting and reporting the activities and outcomes of the hospice liaison program. All of these duties are essential for ensuring a smooth handover to the hospice team.

I have been a Hospice liaison for many years and it has been very rewarding. The one thing I would employ any who is thinking of going into the area of Hospice would be to shadow or volunteer time. The Hospice role can be mentally and physically exhausting especially when you come across multiple deaths in a small period. I believe volunteering will either guide you into this career gracefully or it will tell you where you may not need to be. Volunteering in different area will also give you a greater view of what area you want to be part of.

2 Education and Training

To become a hospice liaison, you need to have a minimum of a bachelor's degree in nursing, social work, or a related field. You also need to have a valid license or certification in your area of practice. Additionally, you need to have at least one year of experience in hospice, palliative, or geriatric care. Some hospice providers may also require you to have a certification in hospice and palliative care, such as the Certified Hospice and Palliative Nurse (CHPN) or the Certified Hospice and Palliative Social Worker (CHP-SW).

Education and training to become a Hospice Liaison can vary from state to state. However, you can become a Hospice nurse right out of school. You can also become a Nursing Assistant to work with clients at home bathing, dressing, feeding, etc. Some people will also shadow to learn the roles and go further to pursue speciality certification

This includes communication skills for communicating clearly and empathetically with patients, families, and health care professionals, as well as interpersonal skills for establishing rapport and trust. Additionally, organizational skills are essential for managing tasks and priorities, as well as problem-solving skills for addressing the needs of patients and families. Furthermore, a hospice liaison must demonstrate compassion and empathy for understanding the feelings and preferences of patients and families, as well as supporting them emotionally and spiritually. All of these qualities are necessary in order to provide quality hospice care.

I think there are certain skills one must have internally and cannot be taught in Hospice, such as empathy, sympathy, and compassion. We as Hospice providers must possess all 3 especially when the family is preparing for a loved one's death. We must also be compassionate in our communication to provide comfort and support to the family and the patient. Empathetic speaking needs to be just a affective as the care given to dying patient.
