Define the scope: Assess how the problem impacts the operations. # Analyse the problem: Form a cross functional team, brain storm & investigate, use the tools like Ishikawa diagram, 5 Why analysis, FMEA. # Identify Root Causes:Validate each potential root cause & contributing factor. # Develop and Implement Solutions:Create Action Plan & implement the changes. # Monitor and Review: Track & monitor the changes and review periodically the effectiveness. # Document and Share Learnings: Record the steps taken and share with the team to prevent similar kind of issues in the future.

En mi experiencia en mantenimiento industrial, considero que lo más importante al realizar un RCA (Análisis de Causa Raíz) es identificar el problema de manera clara y concisa. Esta identificación proporciona datos relevantes para el análisis, lo cual es esencial para encontrar una solución real y definitiva. Tener más datos permite enfocar mejor la solución. Una vez identificadas las soluciones, es crucial detallar un plan y hacer un seguimiento en el tiempo para cerrar el ciclo y evitar la recurrencia del fallo. El monitoreo y control en esta etapa determinan la eficiencia del análisis. Actualmente, la industria invierte recursos en RCA para asegurar procesos confiables y cumplir con los objetivos de la compa?ía.

Para melhorar a confiabilidade de um equipamento, é necessário mitigar todas as falhas, portanto, o primeiro passo é identificar a causa raiz do problema, ou seja, o que de fato é responsável por esta falha. é necessário realizar uma análise para esta identifica??o, pois n?o adianta identificar apenas o que pode ter ocasionado a falha, mas sim, identificar se algo pode ter sido responsável por ocasionar algo que ocasionou a falha e assim sucessivamente, por isso a importancia da identifica??o da causa raiz. Para isto pode ser utilizado por exemplo o diagrama espinha de peixe, que visa identificar falhas relacionadas a máquina, método, m?o de obra, meio ambiente, medida e material.

Para calcularmos a confiabilidade de um sistema ou de um compartimento, além de definir o problema de forma clara, a coleta de dados é uma etapa de extrema importancia, pois se os dados estiverem incorretos estaremos realizando o cálculo de maneira equivocada, fazendo que as tomadas de decis?es aumentem os custos do ativo, quando em casos do n?o aproveitamento do tempo ótimo ou vida útil do sistema. Exemplo: Para um processo produtivo e estratégico de uma empresa a probabilidade de falha máxima a ser assumida é de 18%, mas o processo de coleta de dados n?o segue um padr?o (IT) ou de qualidade, essas coleta pode nos levar a assumir dados reais de probabilidade de falha acima do estabelecido como máximo.

A coleta de dados é um pilar na análise de causa raiz, pois fornece o suporte factual necessário para entender profundamente o problema. Utilizar uma variedade de fontes e ferramentas, como listas de verifica??o e gráficos de Pareto, permite uma abordagem abrangente que aumenta a precis?o das informa??es. é vital também garantir a confiabilidade dos dados para que as decis?es baseadas neles sejam sólidas. Esta etapa prepara o terreno para uma análise eficaz das causas, conduzindo a interven??es mais assertivas e melhorias na confiabilidade dos equipamentos.

The second phase in a Root Cause Analysis involves?amassing pertinent data and proof to substantiate the defined problem. It’s essential to compile details from diverse origins, including documentation, analysis, dialogues, surveillance, gauging and examinations. ? Ensuring the?precision and dependability ?of your data is crucial. Utilize instruments like checklists, process diagrams, and Charts?to facilitate an efficient data collection process. This step is critical in reinforcing your problem definition with solid evidence, leading to a more targeted and effective analysis.

Los datos que pueden ser relevantes para el diagnostico pueden aparecer en situaciones inesperadas, es necesario poner atención a peque?os detalles en entrevistas, inspecciones y ensayos. La mente abierta es la primera herramienta que debe usar un analista de fallas.

Analisar as causas é sem dúvidas o maior desafio quando falamos de ACR. Parece bastante óbvio, mas vai além do senso comum. é normal encontrar ACRs que avaliam somente causas físicas que levaram à falha. Porém, para que a falha n?o volte a ocorrer, é importante também analisar as causas humanas, ou seja, os erros humanos que levaram à falha. E talvez, o mais importante seja analisar a causas latentes, onde as causas humanas se unem as causas físicas, trazendo o principal causador cultural da falha. Atuar somente nas causas físicas pode solucionar o problema inicialmente, mas somente a atua??o nas causas humanas e latentes podem atuar na mudan?a de comportamento e cultura da empresa, minimizando a probabilidade da falha voltar a ocorrer.

Na análise de causas, técnicas como os "5 porquês" e a análise de árvore de falhas s?o essenciais para rastrear as origens dos problemas de forma sistemática. Essas ferramentas ajudam a diferenciar entre causas imediatas, contribuintes e raízes, permitindo uma vis?o clara do que realmente precisa ser corrigido. Essa etapa é crítica: identificar corretamente as causas raiz garante que as solu??es aplicadas sejam efetivas, evitando reincidências e melhorando a confiabilidade dos equipamentos de maneira sustentável.

Luego de tener definido el problema, para analizar las causas es conveniente reunir a las personas que puedan aportar información valiosa para el estudio y determinación de la causa raíz. Con la aplicación de herramientas cómo 5 whys o diagrama de Ishikawa y el aporte de estas personas que conocen las particularidades del proceso, maquinaria, métodos de trabajo, etc, se podrá identificar la o las causas raíz del problema. Una vez determinadas las causas, se deben establecer las estrategias necesarias para erradicarlas.

In this perspective, I would emphasise on one thing, and that is, no possible solution is a stupid solution. Don't go for any solution straight away BUT more importantly, don't rule out any solution without proper brainstorming.

A aplica??o da análise de causa raiz (RCA) é fundamental para melhorar a confiabilidade dos equipamentos, pois permite identificar e eliminar as causas subjacentes de falhas recorrentes. O processo envolve a coleta de dados detalhados sobre as falhas, a utiliza??o de ferramentas como diagrama de Ishikawa e 5 Porquês para determinar a causa principal, e a análise dessas informa??es para desenvolver solu??es eficazes.

Finding solutions also requires skills that are acquired through experiences and attempts. As I said, costs and quality must be taken into account. This is how we can talk about appropriate and effective solutions.

A good RCA process will result in a raft of solutions - not just to the immediate problem but for a host of related causes too. Keep in mind that any solution must be within the power and control of the people/organisations involved. Otherwise it is irrelevant and pointless. A good RCA process also provides opportunity to identify potential solutions that are "high in the tree" that may prevent a host of downstream causes and effects.

This requires?planning and carrying out necessary actions?for the implementation, which may include altering processes, educating personnel, substituting components, or setting up apparatuses. It’s also crucial to?track and evaluate the outcomes?of these solutions and?records any modifications?made. Tools such as?action plans, Gantt charts and control charts?are beneficial in managing and facilitating the implementation process. This phase is vital for ensuring the solutions are effectively put into practice and lead to the desired improvements.

En base a mi experiencia, la aplicación de este método es crucial para evitar tener eventos repetitivos de fallos por no haber identificado las causas profundas del problema. La herremienta que he utilizado ampliamente para esto es llamada ADA: Análisis de Averías, y está compuesta a su vez por una serie de herramientas siguiendo una secuencia paso a paso. Se comienza analizando a detalle lo ocurrido usando 5W + 1H, para luego identificar las causas agrupadas usando el diagrama de Ishikawa o 6M, profundizando posteriormente en cada una de los hipótesis por medio de la herramienta "5 Por Qué 's". En base a las conclusiones que surgen de esta última, se definen planes de acción, los que deberán ser monitoreados hasta su cumplimiento.
