How do you analyze incident data to prevent future accidents?

At this point, collecting this data using a cloud based platform is the only effective means. There are many different tools that can be used to store, aggregate, and analyze the data, but the only effective means of collecting the data in a timely and meaningful way is to utilize an easy to use app that anyone can have on their phone. The data collected needs to inlude near misses and unsafe behaviors (see my comment at the end of this article for disuccsion on leading versus lagging indicators). We need many people on the site participating in this data collection, and it should only take seconds to do.

Accidents happen, but on our sites, they become stepping stones to a safer future. Here's the plan: Data is king. We collect every detail on incidents – date, time, cause, who was involved. Think CSI, but for construction safety! Beyond the basics. We dig deeper, documenting equipment, materials, and environmental factors. Understanding "why" prevents future mishaps. Every voice matters. Reports, interviews, even photos – all angles help us identify root causes and hidden hazards. Accessible data, actionable insights. Secure storage goes hand-in-hand with easy access. A central platform ensures data fuels safety improvements.

Certain hardhats with NFC technology already in place is a good start since an app is typically not required. I think making it the standard in the industry will help accelerate the process also if more data can be entered.

By categorizing incidents, organizations can identify patterns, trends, and commonalities among different types of incidents, which can ultimately help in implementing preventive measures and improving overall safety. One way to classify incident data is by type. Different types of incidents, such as falls, slips, fires, electric shocks, and equipment failures, may require specific interventions or safety protocols to minimize the risk of recurrence.

Critical step. Too often organizations stop short of root cause, leaning toward near cause or technical failure alone. Human and organizational factors need to be considered, as well as a deep dive into the aspects of the management system that should have caught the incident before it occurred.

There are various techniques that can be used to analyze incident data, each providing valuable insights into the underlying factors contributing to the incidents. One commonly used method is the 5 Whys technique, which involves asking "why" five times to delve deeper into the root cause of the incident. By identifying the underlying issues that led to the incident, organizations can implement targeted controls to prevent similar incidents from occurring in the future. Another valuable tool for analyzing incident data is the fishbone diagram, also known as the Ishikawa diagram. This diagram helps to visually map out the various factors contributing to the incident, such as people, processes, equipment, environment, and management.

First and foremost, reporting the incident data helps in communicating the results of the analysis conducted after the incident. By sharing the incident data with stakeholders, you can explain the root causes of the incident, the impact it had on the organization, and the actions taken to prevent similar incidents in the future. This communication is essential for building trust and credibility with stakeholders and demonstrating transparency in your incident management process. Furthermore, reporting the incident data is important for demonstrating compliance with legal and contractual obligations.

And here a common failure is getting too caught up in the lagging indicator (injury rate). The magic comes in the ability to paint a compelling story explaining the events while focusing on what has to be done differently to prevent recurrence(common or systemic cause). If we get management to have their takeaway focused on the change necessary, then we are on our way to a better place.

This may involve implementing engineering controls, administrative controls, or personal protective equipment to reduce exposure to hazards. Engineering controls, such as machine guarding or ventilation systems, are typically the most effective at eliminating hazards. Administrative controls, such as training programs or work procedures, can help reduce exposure to hazards.

All of this advice is great and necessary, but it fails to recognize that it all deals with lagging indicators…information about the past. Root cause analysis of major (and even minor) incidents is important, but needs to be done immediately. The lessons learned also need to be applied immediately in order to affect work moving forward. It’s important to remember, though, that while this is all being done, the job moves on and work continues. Focusing only on lagging indicators often results in fixes being applied too late. Instead, we need to also focus on collecting data that can be analyzed as leading indicators. Frequent unsafe behaviors and near misses often precede minor incidents. Minor incidents precede major incidents.
