How can you use FMEA to improve your problem-solving process?

It an effective tool for identification of risks involved with any activity that is happening in an operational environment. It has wide scope to identify the areas of improvement based on the risk priority number. It can be applied to new sites, products, processes etc.

FMEA is a useful technique to identify and consider risk control measures with the primary goal of improving product quality and reliability. It is important to appreciate that FMEA allows us to consider potential failure modes, one at a time. It is a "bottom-up" activity that starts from a potential failure mode, and identifies underlying cause(s) and effect(s). FMEA is not to be confused with FTA, or Fault Tree Analysis. FMEA cannot help us to see relationships between different failure modes, and how they might occur in a sequence or combination to lead to a top undesirable event such as a hazardous situation or harm. That is where techniques such as FTA or ETA (Event Tree Analysis) are useful.

We've got to keep Two key things in mind, For a successful analysis, gather a diverse team with varied expertise – those experienced team members? They're like gold, boosting accuracy in our assessments and making action plans top-notch. Also, securing a sufficient budget is key. Yes you can focus on the failures and resulting improvements. Or indeed the DVP that many believe is the main output ( a flawed belief in my opinion ). What you will gain most from the exercise is the deeper and broader understanding of the system, that knowledge across the team carries forward in to all aspects of development, indeed transcends the primary goals if the method.

FTA and FMEA should be used as baseline for Product / Process control. Problem related to Product / Process should use the FTA / FMEA as analysis tool. Product / Process that can not be identified in the FTA / FMEA data must be added to the FMEA analysis and be graded in conjunction with the FTA.
