As my experiences in the Oil, Gas, Petrochemical and refinery industries this is hard to follow one specific instruction for analyzing data! For a better understanding of risk management please check ISO 1776 which is a guideline for hazard identification and risk assessment in this industry! Just for quick review, most of the operational risk design in the Hazid study and developed in the engineering phase and developed and reflected in plant HMI programming by mentioning engineering criteria and suppliers equipment or instrument specification and real range. In live plants, one of the main sources of risk is human resources. Just to simplify risk, risks are a step back of NCR and NCR comes from process/Tech, machine and Human!

From what I've seen in the Oil & Gas industry, We always hold several risk management sessions at the early stages of each project. These meetings are usually attended by representatives from all departments representing all stakeholders as much as possible. During these sessions, risks are identified, voted, ranked, combined and/or broken down, and refined along the way. While historical data has immense value, it cannot be the sole source of data/risks. I've seen many projects with unique challenges that had to be identified for the first time. nA combination of both historical data and new perspectives would probably lead to better results.

Change, Risk and Knowledge are the third integrated parts for improvement. Also, following the process approach for updating the instructions and procedures is very important. Sharing the lesson learned with others is the most important part of risk learning and that is part of day-to-day TBM! When we talk about data analysis for risk management that means every single data may affect our activities such as the latest risk in other similar plants or internal or external changes! Using mixed models like COSO and other ISO 31000, ISO 17776, PMBOK, etc. may help to develop creative RBS for companies.
