In our Division of Pain Medicine (which has a heavy focus on Interventional Pain Medicine) we have a double checklist procedure, making sure that at least two different people go through the pre-procedural safety steps in every case. In addition, we foster a policy of “see something, say something”, where we actively encourage our team members (no matter they’re position within the professional hierarchy) to speak up whenever they see something that may put the patient’s safety at risk.
Why do we treat hypertension? To PREVENT complications, such as stroke, among others. No doctor treating hypertension knows the stroke among their patients. They only know the blood pressure. Without knowing the outcomes of our treatments, we can only make minor improvements. With widespread use of electronic health care records, and a robust privacy law (HIPAA), we have the outcomes data, but don't have access to it, or the data analytics for quality improvement. We are too focused on "level I" evidence, where a randomized controlled trial takes almost a decade from inception to publication, yet the data are not applicable to real world patients that don't meet the inclusion criteria of the study. We need help.

While AI will finally be able to analyse all the data that has been gathered and is gathering dust on the hospital hard drives, can it really address the errors made through lack of staff, indequate training and facilities? As Robert said, we are overwhelming an overworked staff with tasks they do not have the time to do at the expense of the patients they care for.
