During this step you should also involve team members and document results in a clear format. To document the update you should prepare and issue a PHA update report that summarizes results, findings, and recommendations. Finally, you should follow up on the update by implementing and tracking recommendations while verifying their effectiveness. It is also important to update related documents to reflect any changes.
