It depends if formal reporting provides a receptacle for each of the above mentioned reporting options. If not, the individual would need to find informal platforms (e.g. casual get-togethers, short message service etc) to execute reporting types that are not captured at formal platforms (appraisal, official meetings etc)

Do you want them to need to make a HR or business decision? Are you hoping they share the information to their own stakeholders? Then, how would they need the information to be packaged and delivered in their preferred manner, so they can execute the action you want them to take? Simple, direct reports work better than complex, colourful ones with fluff.
