How can you establish clear goals and objectives for interprofessional collaboration?

Every health profession needs a representative at the table to set the goals collaboratively. If one engages the professional team in goals, concept, and design, the goals will have strong multidimensionality.

Assessing the situation for interprofessional collaboration should include identifying a team lead or care coordinator who can prioritize patient-centered and patient-led care. When assessing needs and using tools like SWOT it’s critical to understand scope limitations and which specialties or providers are necessary and what their scope is. When multi-disciplinary teams are involved there will be instances of miscommunication and schedule conflicts. Having a team lead or care coordinator who is educated and equipped to know who to talk to for which concern makes a difference. Also understanding contrasting care models is necessary (e.g. school-based providers do not follow the same care models as community-based providers).

When working with organizations, one of the most common problems I come across is not having clearly defined goals that are measurable. If you don’t know what you are aiming for, how do you know if you are successful?

Define Purpose: Clearly articulate the purpose and desired outcomes of the collaboration. Identify Stakeholders:Identify key individuals or groups involved in the collaboration. Clarify Roles and Responsibilities: Clearly define team member's role, responsibilities, and expectations. Establish Communication Protocols: Define how communication will occur within the team. Build Trust and Rapport: Foster a positive and open team culture. Regularly Evaluate Progress: Schedule regular check-ins to assess progress, discuss challenges, and make any necessary adjustments to goals or strategies. Celebrate Achievements: Recognize and celebrate milestones and accomplishments. Seek Feedback: Encourage open feedback from team members.

Interprofessional collaboration (IPC) and person-centered care are complementary pillars in the edifice of modern healthcare, together forming a dynamic alliance focused on enhancing patient outcomes and experiences. At its core, person-centered care places the individual at the forefront, recognizing their unique needs, values, and preferences. In this context, IPC becomes the orchestration of a harmonious symphony among diverse healthcare professionals, each contributing their specialized expertise to cultivate a comprehensive and personalized approach to patient care.

The intention or commitment to increase IPC is admirable but in practice it is also insufficient. Setting SMART goals is critical to achieving interprofessional collaboration (IPC) because SMART goals ensure clarity. To set a SMART goal you must have a clear understanding of the intended outcome as well as the specific tactics required to achieve the goal. SMART goals also specify which parties are responsible for a given activity. In effect, a well-written SMART goal provides all stakeholders with the road map to success, provided that it is followed by execution. In my experience, execution is often where the lapse occurs.

A lot of situations can be improved by a simple 5-10 minute in person conversation. Especially when dealing with people/patients. Not everything can be charted or e- mailed. Some things or information must be shared one on one.

Simply reaching out in person can really get you the information you need. Everyone is overwhelmed with emails at work and some of us don’t have the time to give our emails the attention they deserve. Come walk the walk and come in with an open mind. You’d be astonished the things you can learn by simply listening and looking.
